Early in our history, Medford Leas charted a distinctive course in health care delivery — significantly different than the path followed by most CCRCs. Pivotal to this approach was the decision to commit the necessary resources to ensure that the team of health care professionals designing and directing our program would be our own, full-time, on-site staff, lead by a full-time medical director, and two full-time nurse practitioners. This innovation led to the uniquely comprehensive, integrated health care system our residents have been enjoying and thriving on for nearly 30 years.

What it means to move through the continuum of care”
Residents move to Medford Leas’ residential homes and apartments while they are still active and in good health. If and when a resident’s health status undergoes a permanent change, a transfer either to an Assisted Living or Long-Term Nursing Care accommodation may be deemed necessary. Decisions to transfer a resident to areas of the community which provide 24-hour a day care are always made with greatest care and respect for the individual and always include the following steps:
- extended consultation between an interdisciplinary team and the resident (or the resident’s designated surrogate decision-makers, if appropriate);
- consideration of all alternatives; and
- determination that the resident’s health care needs cannot be served adequately at the resident’s present location.
