CONTINUING CARE RETIREMENT COMMUNITY

Continuing Care Retirement Communities combine attractive residences with the security of a coordinated system of services designed to address your needs as you grow older. Not all CCRCs offer all elements of the continuum. Medford Leas provides a full continuum of care, including primary (physician) care, companion services, home care, respite care, assisted living, memory support, short-term rehabilitation, and hospice.

Medford Leas provides varying levels of specialized services in separate neighborhoods of assisted living support including various levels of personal assistance and memory support.
REHABILITATION

SHORT TERM REHABILITATION

We offer Medicare-certified, skilled nursing services for residents recovering from illness or returning from surgery. Our in-patient treatment philosophy is to assist you in realizing your highest potential. The team will work with you to set and achieve realistic goals, both for the immediate and long-term future. We offer physical, occupational and speech therapies, IV therapy, wound care, pain management, and nutritional management.

HOME CARE

CARE WHERE YOU ARE MOST COMFORTABLE

Medford Leas also offers an accredited home care services, including nursing and companion services to residents in our Courtyard Apartments. Our goal is to keep you as independent as possible, as long as possible.

PHYSICIAN SERVICES

PHYSICIAN (WELLNESS) SERVICES

Unlike most CCRC’s Medford Leas employs its own medical staff. A team of physicians, advanced-practice registered nurse, RNs and LPNs are available 7 days a week to respond to your needs. Appointments are readily available and ample time is given to each resident. In addition to primary care services, several other medical services are available on-site including: podiatry, dentistry, psychiatry, psychology and a pharmacy.
